李連英（黎耀祥飾）家境貧窮，父親沉迷賭博，最終欠下一身巨債，為救家人，連英唯有淨身入宮當太監，後跟隨咸豐的親信掌璽太監劉多生（鍾景輝飾）辦事，並從多生身上學到紫禁城中的生存之道，亦培養出他心思縝密，處事謹慎的性格。樂天知命的連英做事規行矩步，不求有功，但求無過，甘心留在景仁宮（冷宮）當個無品隨侍太監。
與連英自小入宮的還有安德海（曹永廉飾），德海自幼失去雙親，生活困苦，更慘被販子賣至宮中當太監，令他滿心不忿。德海自此立志要用子孫根換取飛黃騰達的生活，遂千方百計努力成為慈禧（米雪飾）的掌案太監。及後咸豐駕崩，肅順把慈禧、慈安（邵美琪飾）和同治（梁烈唯飾）幽禁在熱河行宮，德海幸不辱命，拼死把密旨送回北京予恭親王（張國強飾），最終慈禧得以奪回大權，而德海亦立下大功，大受慈禧寵愛，繼而成為慈禧身邊炙手可熱的首領太監。
德海與連英雖然地位懸殊，但兩人情同手足，德海欲提攜連英到長春宮，不過在連英心目中，慈禧太后專橫霸道，奴才動輒得咎，所以連英對慈禧敬而遠之。…
